ABOUT

Catholic Charities Partners in Housing program serves individuals and parents and their children, who have a disability or who have been chronically homeless in Bernalillo County. Participants receive assistance from Case Managers in obtaining housing, disability benefits, counseling, child care and other resources.

The focus for participants in this program is to build a stable home for themselves and their children, while they obtain the appropriate services that will lead to their increased well-being. Applicants must have a documented disability (mental or physical) and proof of  Chronic homelessness. Participants with a substance abuse disorder will also be assisted in obtaining Housing.

Catholic Charities does not handle their own housing referrals for Permanent Supportive Housing. Referrals are handled by:

People who are chronically homeless (on the street or in a shelter for one year or more) and have a documented disability are eligible for this program and must be referred through the Coalition to End Homelessness. 

Referrals are handled through the Coalition to End Homelessness. Please call (505) 772-0547 (Santa Fe) or (505) 217-9570 (Albuquerque) to take the Coordinated Entry Assessment over the phone. 

Permanent Supportive Housing has no expiration for those who receive this voucher. 

Proof of Chronic Homelessness includes length of time homeless. (One complete year Chronic Homelessness proven, or four separate occasions within the past three years that are equal to one full year, with Written documentation as well as a documented permanent disability that seriously impairs one’s ability to work full time.)
